Our clinic team offers health checks, ongoing care for long-term conditions, and medication reviews to help whānau stay well and in control of their hauora.
A hauora check is a 15-minute appointment with a tākuta designed to give whānau an overview of their overall hauora. It’s an opportunity to kōrero about their hauora, ask pātai, and discuss any concerns whānau may have.

Long-term condition management supports whānau living with ongoing health conditions such as mate huka (diabetes), heart disease, asthma, COPD, high blood pressure, kidney disease, and other chronic health needs.
At Te Manu Toroa, a whānau-centred care team works alongside whānau to create personalised plans that support them to live well, stay well, and confidently manage their condition.

Tautoko is available for both newly diagnosed and ongoing long-term conditions.
Medication reviews help ensure whānau are taking the most appropriate medicines and know how to take them safely, particularly when managing multiple medicines or a long-term condition.
Our Mātanga Rongoā (Clinical Pharmacist Services) are part of the whānau-centred care team, working alongside tākuta (doctors) and nēhi (nurses) to support safe and effective use of medicines for whānau.

A medicine review includes a review of all medicines being taken, including prescription, over-the-counter medicines, supplements, and rongoā Māori.
Appointments can be booked directly with Clinical Pharmacists or through your tākuta or nēhi.
Whānau can access these clinical services by making a booking via the MyIndici app, by phone, by email, or by visiting reception.
Some services may only be available for enrolled whānau.
