Our Home Based Support Service (HBS) walks alongside adults with mental health challenges, supporting recovery while staying close to home and hāpori.

This free, community-based service supports individuals experiencing mental health challenges. Our kaimahi offer person-centred support for those living independently, helping them build resilience, strengthen their hauora, and stay connected to their whānau and hāpori.

Even if a referral is not made directly by the whānau, your family GP will be kept informed and involved as the primary medical caregiver.
Whānau can access this service through referrals from themselves or a whānau member. Referrals may also come from GPs, Needs Assessment / Service Co-ordination (NASC), community mental health teams or acute mental health inpatient services.
This service is available Monday–Friday, 8.00 am–4.30 pm
