Looking after your hauora doesn’t have to be hard. Our community-based Dietitian Service provides whānau-centred support to help you use nutrition to manage your health conditions and improve your hauora.
This service is specifically for conditions such as Diabetes, Obesity, and Heart Disease. Check if you’re eligible here.
Our support includes:
Visits can take place at your whare, mahi, hospital, one of our GP clinics, or any location that works best for you, for short-term or ongoing support.
This service is available for whānau who meet any of the following criteria:
Whānau can access this service through referrals from GPs, specialists, community organisations, other health professionals, whānau members, or by referring themselves.
This service is available Monday–Friday, 8.00 am–4.30 pm
