What This Service Offers

Our registered nēhi provides community-based support for rangatahi and adults living with heart conditions. Our Community Nursing Services include:

  • Cardiac assessments and monitoring
  • Education and guidance to manage heart conditions
  • Support with blood pressure, cholesterol, and overall heart health
  • Development of personalised care plans
  • Advocacy and assistance at specialist appointments in clinics or hospitals
  • Referrals to other healthcare services

Visits can take place at your whare, mahi, or one of our GP clinics, for short-term or ongoing support.

How to Access the Service

Whānau can access this service through a referral from a GP or other healthcare professional. If you’re interested, please talk to your tākuta or nēhi.

This service is available Monday–Friday, 8.00 am–4.30 pm
